Position Specifications
 Staff Nurse Per Diem - Home Care Services Home Health

This is a Per Diem position with variable hours and variable shifts. There is no guarantee of hours worked.  This position is NOT eligible for benefits. 

Salary Scale: Scale starts at $25.01. Placement within the scale is dependent upon years of work experience.  The salary scale is reviewed on an annual basis for market comparison and may change with the market.
 Responsibilities
 The Registered Nurse is responsible for providing nursing care adapted to individual patient needs based on the nursing process, which includes assessment, planning, implementation, and evaluation.  He or she is also responsible for delegation and supervision of personnel, as states above, that participate in the provision of care to ensure that patient needs and expected outcomes are met.  This position is a member of the hospice interdisciplinary team that assesses, plans, implements and evaluates all aspects of patient care for terminally ill patients and their families.  Responsible for providing assessment, nursing diagnosis and developing and managing a hospice plan of care.  Provides training, orientation and education of staff.
 Qualifications
 Graduate of an accredited Registered Nurse Program.  Bachelor’s Degree in Nursing preferred.

Current license to practice as a Registered Nurse in the State of Washington with annual renewal required.  Current BLS Certification.  Certification as hospice nurse preferred.  Valid driver’s license is required.  In the case of a newly-relocated individual, a Washington state license must be acquired under the circumstances and within the required timeframe set by the state of Washington.  Possession of the minimum (or greater) insurance coverage as mandated by the state of Washington also required.  When a personal vehicle is to be used for business purposes, a valid vehicle registration form is required.

Experience acquired from Accredited Registered School of Nursing.  Minimum of one year experience in acute care nursing facility required.  One year of experience in home health, hospice or hospice inpatient facility preferred.  Excellent skills in pain and symptom management, grief and loss communications, and working knowledge of infusion pumps preferred.  Requires current Washington State Driver’s license, use of automobile and current automobile liability insurance.  Ability to perform a wide variety of nursing functions in hospice care provided in a variety of settings including home, residential and acute care.  Ability to perform assessments, develop an interdisciplinary plan of care for patient/family unit, provide direct nursing care and education and orient families and staff.  Skilled to work with a wide range of staff as part of an interdisciplinary team including physicians, nurses, social workers, spiritual counselors, grief counselors, volunteers and other ancillary staff.  Able to read, write, understand and communicate in English.  Ability to complete mathematical calculation (addition, subtraction, multiplication, division, equivalencies).  Must have knowledge of human growth and development to modify services to the age and development stage of the patient.
